Background and Aim: Neonatal mortality rate is one of the main indicators in health care systems. 60% of deaths in this age group belong to premature neonates. In spite of reduction in premature neonatal mortality rate in recent years, it is still a health problem in social health management. In this study, causes of neonatal deaths are assessed in Shohadaye Tajrish hospital during 3 years November 2004 to June 2007. Materials and Methods: This study is a descriptive study based on the neonatal files data, which performed to determine proportional mortality in dead neonates. Results: According to the results of this study, 60 neonates died in the mentioned period in Shohadaye Tajrish hospital, among them, 44% were female and 55% were male and 1% with undetermined sex. 59% of infants were born by caesarean section and 41% were born by normal vaginal delivery. The Apgar score in 63% of neonates was less than 7. The birth weight in 27% of infants was less than 1000 g, 50% between 1000 to 1499 g, 8% between 1500 to 1999 g, 8% between 2000 to 2499 g, and 5% were over 2500 g. Gestational age in most of these infants was between 28 to 31 weeks. Most frequent mortality cause was respiratory distress syndrome (RDS) and then sepsis, congenital anomalies, asphyxia, metabolic disorders, pulmonary hemorrhage and necrotizing enterocolitis. Conclusion: Concisely, the most frequent causes of death in neonates were RDS and Sepsis, so prophylaxis and appropriate treatment of these diseases are very important for decreasing of neonatal mortality. As caesarean section was the main way of delivery of died neonates, avoiding unnecessary caesarean section may be effective for decreasing neonatal mortality. Because most of died neonates referred from another hospitals providing neonatal intensive care unit (NICU) facilities in delivery centers can be very effective on survival of neonates
