Investigating the relationship between the level of CRP with PPROM and cervical length in pregnant women who visit in the prenatal care clinic of Dr. Shariati hospital of Tehran

Background and Aim: PPROM is defined as the preterm premature rupture of membranes before the initiation of delivery and before the 37th week of pregnancy. PPROM has many maternal and fetal complications and is primarily generated by infection. This infection can bring about changes in cervical length at the incipience of pregnancy. Since CRP levels increase in blood in response to infection and inflammation, the aim of this study is to investigate the relation between mothers’ CRP level as a marker of infection with cervical length and the occurrence of PPROM. Materials and Methods: 418 pregnant women with singletons who had visited the parental clinic of Shariati hospital participated in this descriptive study. Their CRP level was measured based on mg/dl between the 6th and 20th weeks of gestation and their cervical length was specified with vaginal ultrasound based on mm between 14 to 20 weeks of pregnancy. In this study the relation between the CRP level and the incidence of PPROM with cervical length was statistically determined via the t-test. Results: Out of 418 pregnant women 11 (2.6 percent) had PPROM and their CRP level was 22.5 mg/dl±2.8. The remaining 407 people who didn't suffer from PPROM had a CRP level of 8.73 mg/dl±4.5. In this research there was a significant relation between CRP level during the first 20 weeks of gestation and the incidence of PPROM (P<0.000). Furthermore, the CRP level in people who had a cervical length of 25mm or less was 21mg/dl±4.8. It was 8.5mg/dl4.2 in people who had a cervical length of greater than 25mm. The study found a significant relation between the mothers' CRP level with a cervical length of 25mm or less during the first 20 weeks of pregnancy (P<0.000). ‍Conclusion: It seems that the CRP level is associated with the incidence of PPROM and cervical length and that the CRP level as a marker of inflammation and infection is higher in people affected with PPROM and those who have a cervical length of 25mm or less. To investigate this further, it’s recommended that a cohort research be studied.
