fa بررسی عوامل مؤثر بر بروز و عدم گزارش‌دهی خطاهای دارویی از دیدگاه پرستاران پزشکی Medicine پژوهشی Original <p dir="RTL"><strong>چکیده</strong></p> <p dir="RTL"><strong>سابقه و هدف: </strong>خطاهای دارویی ایجاد شده توسط کارکنان مراقبت سلامت، خطر جدی را متوجه ایمنی بیماران می‌نماید. اولین قدم برای ارایه راه‌کارهای پیشگیری از این رخدادها، بررسی علل و چگونگی گزارش‌دهی آنها است. هدف از این مطالعه، تعیین عوامل مؤثر بر بروز و عدم گزارش‌دهی خطاهای دارویی از دیدگاه پرستاران در بیمارستان‌های منتخب شهر تهران بود.</p> <p dir="RTL"><strong>مواد و روش‌ها:</strong> مطالعه‌ی توصیفی- مقطعی حاضر در سال 1394 صورت گرفت. جامعه‌ی پژوهش، پرستاران 4 بیمارستان منتخب شهر تهران بود که با توجه به فرمول حجم نمونه،200 نفر از آنان انتخاب شدند. ابزار گردآوری داده‌ها، پرسشنامه‌ای استاندارد شامل بخش اطلاعات دموگرافیک پرستاران با 10 سوال و موانع عدم گزارش‌دهی خطای دارویی از دید آنان با&nbsp; 30 سوال بود.</p> <p dir="RTL"><strong>یافته‌ها:</strong> میانگین سنی پرستاران 8/39 سال و همچنین میانگین سابقه‌ی کار پرستاران معادل 9/9 سال بود. فراوانی موانع مربوط به فرایند گزارش‌دهی خطاهای دارویی در پرستاران نشان داد که&nbsp; اهمیت نداشتن گزارش‌دهی بعضی از اشتباهات دارویی از نظر پرستار با میانگین 8/2 بیشترین و فراموش نمودن پرستار در زمینه‌ی گزارش اشتباه دارویی با میانگین 6/2 کمترین تأثیر را داشت. موانع مدیریتی عدم گزارش‌دهی خطاهای دارویی، تمرکز مدیر پرستاری به شخص اشتباه کننده با میانگین 3/3 و نامناسب بودن واکنش مدیران پرستاری با میانگین 1/3 به ترتیب بیشترین و کمترین تأثیر را داشتند.</p> <p dir="RTL"><strong>نتیجه‌گیری: </strong>ارایه بازخورد در رابطه با خطاها، زمینه‌سازی برای تناوب گزارش‌دهی حوادث، تبادل اطلاعات مربوط به بیمار و همچنین آموزش پرستاران در رابطه با خطاهای دارویی در بیمارستان‌های تحت مطالعه جهت ارتقای فرهنگ ایمنی بیمار و کاهش خطاهای دارویی، ضروری به نظر می‌رسد.&nbsp;&nbsp;</p> <p><strong>Abstract:</strong></p> <p><strong>Background and Aim:</strong> Medication errors caused by healthcare providers can seriously threat patients&rsquo; health. The first step to present strategies for preventing such incidents is to assess the causes of errors and no reporting. The purpose of this study was to investigate the causes of medication errors and lack of their reporting from the perspective of nurses working in hospital.</p> <p><strong>Materials and Methods:</strong> This study was cross-sectional and conducted in 2015, and nurses community in Shahid Beheshti hospitals were studied.&nbsp; A sample of 200 nurses selected according to the formula of the sample size and survey instruments and consisted of three parts: demographic information, types of medication errors and barriers to reporting medication errors. Test-retest test was equal to 86%.</p> <p><strong>Results:</strong> The average age of nurses participating in this study was 39.8 years old and the average work experience of nurses was 9.9 years. Most barriers related to medical error reporting process in nurses lack of importance of reporting from a nurses perspective with an average 2.8 and nurse forgets to report medication errors with an average 2.6 the lowest impact.
